Sanctuary Health is currently looking for an experienced and dedicated band 5 community nurse to work in Nottingham.

The ideal candidate will possess highly developed specialist knowledge of nursing and be registered with the NMC.

The pay rate for this locum nurse job is £19.00 per hour (LTD Company Equivalent) for weekdays, £24.24 per hour (LTD Company Equivalent) for Saturdays and Nights, £29.79 per hour (LTD Company Equivalent) for Sundays and Bank Holidays and is an initial 3-month contract.

Role Purpose:
- To provide high quality holistic evidence based care to patients / service users that is responsive, effective and efficient
- To facilitate and support timely discharge and prevent inappropriate admissions by supporting patients / service user to remain as independent as possible and maintain or improve their level of function and quality of life.

Role Context:
- To support the re-design of Adult Community Health Services within Nottingham West to achieve a model of wrap around care that is responsive to the needs of the patients / service users
- To be pro-active in delivering anticipatory and responsive care, promoting independence, health life style, reduction in health inequalities to improve the overall quality of life in the local population
- To offer a comprehensive nursing service to the resident population of Nottingham West, providing a multi-disciplinary and multi-agency approach to maximise patient function, well-being and self-care planning

Key Accountabilities:
- To target people who would otherwise face unnecessarily prolonged hospital stays or inappropriate admission to acute inpatient care, long term residential care or continuing NHS in-patient care.
- To deliver high quality care to patients / service user in the community and clinic setting
- To work effectively within the Multi-Disciplinary Team (MDT).
- To work at all times within the Codes of Professional Conduct.
- To work flexibly and innovatively to meet the needs of the registered population
- Treat patients / service users as individuals
- Contribute to the public health agenda by raising awareness of health and well-being needs to individuals
- Contributes to the service development to support the service re-design
- Maintain accurate and contemporaneous documentation to ensure timely, effective communication and data collection.
- Ensure that patient / service user documentation demonstrates clearly the organisational Guidelines for Records and Record Keeping.
- Be personally accountable for maintaining skills, knowledge and competence in the use of SystemOne
- Be aware of the Data Protection Act to ensure appropriate action is taken to ensure confidentiality and the protection of patient / service user identifiable information at all times.

Requirements of the nurse:
- An in date DBS is required for all roles/work
- RGN qualified
- NMC qualified
- The candidate must have their own transport and/or ability to travel- *Travel paid at 24p per mile*
