    Sanctuary Health is currently looking for a band 6 community psychiatric nurse to fill a full-time role in Mitcham.

    The pay rate for this locum CPN job is £27.33 (LTD Company Equivalent) per hour. This job is an initial 3-month contract.

    Responsibilities of the CPN:
    - To signpost service users to other agencies/services and/or discharge to primary care where necessary
    - To manage a caseload of clients with complex needs, using evidence-based/client-centred principles to assess, direct treatment and discharge plans
    - The post holder is expected to utilise community resources, working closely with other elements within Merton and Secondary services
    - The post holder will ensure user and carer involvement in all aspects of service delivery
    - The post holder will be expected to promote, by example and leadership, a high standard of patient care
    - To be responsible, under supervision, for the coordination and transfer to the relevant treatment team of a designated caseload of services users with enduring mental health problems or service users who meet the threshold for primary care support.
    - To maintain a high standard of individualised client care, recognising factors and events that are likely to affect the mental, emotional and physical wellbeing of clients/carers and provide advise and support when required.

    Requirements of the CPN:
    - Valid NMC registration under mental health
    - Experience in working in a community mental health setting is essential
    - A degree or equivalent within mental health nursing (RMN Trained)
    - Diploma/BSC in Community Nursing
    - Experience of working in a multidisciplinary team
    - Car owner/driving license with no endorsements
