Sanctuary Health has a job vacancy for a Band 6 Community Physiotherapist to work in a Discharge to Assess Service in Hitchen. The ideal candidate will possess highly developed specialist knowledge of Physiotherapy and be registered with the HCPC.

The pay rate for this locum Physiotherapist job is £23.50 to £24.50 per hour (LTD Company Equivalent) and is an initial 2-month contract.

Requirements of the Physiotherapist:
- Degree or equivalent within Physiotherapy
- HCPC registered as a Physiotherapist
- At least 2 years post qualifying experience within Physiotherapy
- Must be a car driver

-To deliver a high standard of patient care using the accepted evidence-based framework.
- To set and maintain high standards of care and provide support to the wider DH2A Multi-Agency Team in both clinical health and social care.
- To take responsibility for patient/service user initial assessment (within 72hrs of discharge), planning, implementation and evaluation of individualised therapy care to patients living in the community.
- To participate in the long term management of patients within the locality and work in collaboration with other locality team members.
- To take continued responsibility for a defined caseload.
